Day Centres

Day services provision for older people is made up of different centres that offer a wide range of services with a variety of support needs. People are individually assessed in order to ensure that the centre they attend is most appropriate to them.

The service aims to:

  • Provide rehabilitative, therapeutic and recreational services tailored to the individual
  • Actively promote and encourage involvement and independence from older people attending the centres
  • Ensure regular monitoring and reviewing
  • Contribute in maintaining the person in the community
  • A service that is cultural sensitive and reflects the diversity of Newham.
  • Offer appropriate support and advice to families, relatives and friends


Who can use this service?

Day services are available as part of a package of care tailored to the individual. To be eligible for day services you must be referred by a social worker in line with the Single Assessment Process.

This will establish if you meet the Fair Access to Care Services Eligibility Frame work for Adults Social care services. The centres listed are funded by the London Borough of Newham


How can I access this service?

  • If the person being referred has an allocated social worker please contact them in the first instance. They will then arrange to assess the person wishing to attend the centre and complete a carer's assessment
  • If you do not have an allocated social worker but are in contact with one of our social work teams, please contact them
  • If you do not know if the person being referred is known to a social worker team or they do not receive any other services please contact the Initial Contact Service who will arrange for an assessment to be carried out. 020 8430 2000 Extension 88004 social services hotline Monday to Friday form 9am to 5pm


What services do you offer at the day centre?

A wide range of services is available. As far as is possible we try to ensure that all service users have a choice in the services they receive at the centre, and that the services are tailored to the specific needs of the individual.

These include:

  • A wide ranging activities programme
  • A program of workshops, theme days and professional entertainment
  • An allocated member of staff who is responsible for the service users well being
  • Breakfast, lunch and a variety of hot and cold drinks
  • Door to door transport (the provision of transport will be based on an assessment of individual need)
  • Drop in service - this service is available on Sundays, subject to prior arrangement with a social worker, if an individual has a diagnosis of Dementia or Mental Health support needs


Involvement of families, relatives and friends

A large number of people would be unable to remain in their homes without the support from families, relatives, friends and neighbours. It is vital that they are able to access the relevant information and advice when trying to find out which services are available.

The day centres can offer:

  • A regular line of communication with an identified member of staff
  • To act as an advocate when dealing with other services and professionals
  • To listen to the families, relatives and friends views in the support given to the person attending the centre
  • Regular review meetings
  • To make appropriate information available


Transport

Newham`s Environment Services run the transport. The transport section provides a door-to-door service, with an escort providing appropriate support. All vehicles are fitted with seat belts, tail lists and have spaces for wheelchair users.
