The Community
Involvement Team
The Community Involvement Team helps
people with learning disabilities to be involved in their
community.
We work with adults with learning disabilities and
support the transition process for younger people as they become
eligible to access adult services and opportunities.
Our focus is on helping people with learning disabilities live
healthy and independent lives. This may mean providing support
with:
- Relationships and friendships
- Employment
- Health
- Leisure
- Education
- Empowering them to make choices about the lives they want to
lead
What we do
We provide Person Centred Planning, a kind of
planning that has the individual at its heart. We work with the
individual with learning disabilities to discover their dreams and
ambitions for the future. We then support them in finding creative
ways of planning ahead. Key people in their lives are brought
together to build a plan.
The Person Centred Planning co-ordinator works across the borough
linking with organisations, carers and service users to support,
implement and promote Person Centred Planning.
The Community Builder works to link people with
learning disabilities with the community in various ways. For
example, it may be a project that brings various people from the
community together. The recent journey project was about raising
awareness around people with learning disabilities using public
transport. We brought young people, people with learning
disabilities and people from NewVic (a local sixth form college)
together in a drama project focusing on transportation. This was
used as part of the induction process for bus drivers in
Newham.
Planning Circles is an approach of Person Centred
Planning. It brings together families, friends and professionals to
help the individual with learning disabilities plan for the
future.
