Reablement case study

Supporting CE* to Regain their Independence

*CE is one of our resident's whose identity has been annoymised.

After spending nearly eight weeks in hospital, CE was referred to Newham’s Reablement Service by the hospital social work team. Understandably, CE felt uncertain about how they would manage at home again. The reablement team stepped in to offer tailored, compassionate support designed around CE’s personal goals and strengths.

Together with CE, the team co-created a plan focused on what mattered most to them. The goals were:

  • Feeling confident in managing personal care
  • Keeping their home clean and comfortable
  • Taking medication safely and independently

Support workers gently guided CE through daily routines, encouraging them to take the lead while ensuring safety and dignity. This included:

  • Assisting CE in the bathroom and helping them feel secure using new equipment
  • Supporting CE to dress in a way that was easier, looking at clothes with less fastenings
  • Encouraging CE to take pride in their home environment, using a rest and recovery approach to manage daily tasks

To help CE manage their medication, the team carried out a self-medication assessment and introduced blister packs, which made things simpler and more empowering.

By the end of the reablement period, CE had met all their goals. Reflecting on the experience, CE shared:

“I had just come out of hospital after being in for about eight weeks. I don’t think I would have managed without the reablement service. The staff encouraged me to do household chores, and now I can do them myself.”

 

“The service is absolutely brilliant. I’m now able to wash and dress myself thanks to the encouragement and support I received.”
