Annual Health Checks

If you are over 14 and have a learning disability, you should get a health check from your GP every year.

If there's anything the doctor thinks you could do to be healthier, they will make a plan with you with some ideas on what you can do to be healthier. This is called a Health Action Plan.

Hospital passports

A hospital passport tells the hospital about you (your learning disability, your health, your likes and dislikes and how you like to communicate).

When you're in hospital, you can show it to doctors and nurses so they know all about you and how to make things easier for you.

The Newham Health Team for people with learning disabilities

The Newham Health Team for people with learning disabilities provides specialist health care to people with severe or profound and multiple learning disabilities.

They also support people with mild/ moderate learning disabilities to access mainstream health services.

The team provide services for people:

  • Over the age of 18
  • Who have a diagnosed learning disability
  • Who are unable to have their needs met by a mainstream service without support.

They can continue working with people even if they are over the age of 65 if their primary needs are related to their learning disability rather than their age.
