Young Adults Preparation Team
The Young Adults Preparation Team is a specialist team supporting and safeguarding young people from 18 years to 24 years who:
- have a diagnosis of a learning disability; learning disability and autism; or a learning disability and a physical disability
- have a mental health challenge or other disability and have been receiving support from children’s services in Newham
The team's role is to support young people and their families, to build sustainable long-term solutions to their care and support needs by focusing on enablement and independence. All work is person-centred and strengths-based. This means the young person and their family play an active role in developing a support plan which will enable them to achieve their aspirations and be part of their community.
The team's role includes carrying out Care Act Assessments, Mental Capacity Assessments, Community Treatment reviews, Adult Safeguarding, working jointly with the Care Leavers Team, supporting colleagues in SEND Education to develop Education Health Care Plans, working with carers and Court of Protection work.
This team was set up to ensure the best possible experience and outcomes for young people moving from children’s to adult services. Close partnership working with our children’s colleagues, and early involvement from adult social care ensures a smooth transition for the young person and their families on their journey into adult services and navigating early adulthood. We work with young people and their families to feel confident in their next steps and this important life change.

The team work with young people who are eligible for adult social care.
